          Provide official high school transcript, copy of GED, or

           official college transcript

          May be required to take Accuplacer for placement purposes

          Provide test scores (TAKS, THEA, COMPASS, ASSET, ACT or

           ACCUPLACER)

          Provide current audiological records

          Foreign students must take TOEFL or equivalent
